Before the incident

The Teammate was assisting in setting up to run a new batch of tubes. (calibrating timing of cutt-off)

What happened

When doing a mill set-up tubing comes off the mill with an open seam. this tubing is cut and dumped towards the back of the mill into our scrap bay. During this procedure the teammate misplaced his hand and got caught within the pinch point of the dump table and a sliding tray.
