Before the incident
Maintenance was called to remove a piece of stuck gating on the skirting on the side of the [REDACTED] [REDACTED]. After performing Lockout Tagout on the conveyor [REDACTED] climbed onto the conveyor and located the stuck gating (scrap from the cast iron molding process).
What happened
[REDACTED] used a sledgehammer in an attempt to free the stuck gating. He swung three times at the gating; all below the knee at ankle height. On the third swing the gating was freed from the skirting of the conveyor with force making contact with his lower left leg.
