What happened
A Shipping helper was moving stacks from the stacker machine to the shipping trailers. The employee picked up a stack of Good Seed 2PK thin Slice finish product (308 lbs.) with the dolly and started pushing it towards the dock area. The employee did not have proper control of the stack so it was set down. Immediately after the employee attempted to pick up the stack with the dolly and was unsuccessful. Then the employee put their right foot on the dolly wheel and their left foot on the dolly axle bar to use their body weight and tilt the stack on the dolly. The stack tilted and due to the improper body mechanics the employee fell backwards with the stack and the dolly
