When walking back through the area the employee tripped over the anchor bolt with the right foot. The employee fell to the floor from standing height and tried to brace with the left hand and still hit the ground on the left shoulder. No other body parts struck the floor. Now experiencing pain in the left elbow joint and pain radiating in the surrounding muscles.

Engineering techs removed Line #4 bridge conveyor support in order to fit scissor lift trhough the area to change a light fixture. This exposed anchor bolts that fasten the conveyor support to the floor. When walking back through the area the employee tripped over the anchor bolt with the right foot. The employee fell to the floor from standing height and tried to brace with the left hand and still hit the ground on the left shoulder. No other body parts struck the floor. Now experiencing pain in the left elbow joint and pain radiating in the surrounding muscles.
