He was in the process of changing a tool out of a machine.The tool was suspended from the crane and he was raising it to loosen a water fitting under the tool. He had left a socket wrench sitting on top of the tool.

After raising the tool he goes to loosen a water fitting from under the tool. He had forgot about a socket wrench he had left on top of the tool. As he started loosening the bolt the wrench fell from the top of the tool and hit him on the left side of his eyebrow.

The injury was a 1cm laceration to the left side of his eyebrow.
