On [REDACTED] EE informed management as the pain has become worse. EE refused to work with trainer stating that they had worked with them before in [REDACTED]. EE stated that he had this pain before in [REDACTED] didn’t report it at work but received PT through his personal physician. EE was adamant about going to Concentra. He went to the clinic and received restrictions for a strain.
