Before the incident
Removing rock from the roof
What happened
[REDACTED] was removing rock from the roof using the vac. The PVC hose was extended out the length of the roof and they were having issues from time to time with the hose starting to plug. When the plugging begins to start [REDACTED] would pick up the hose at the joint locations to be able to get a little flexability in maneuvering to allow the rock to move and free up and exhaust out. During the time the injury took place [REDACTED] grabbed the hose at the joint wiggled it to loosen the rock and then let the hose drop back down to the roof. As the hose dropped the excess metal from the tightened clamp sliced through his glove and penetrated his right index finger.
