What happened
While EE was at account CVS #9212 they were cleaning our product from the racking. While clearing out this product EE was on all fours handling product on the 2nd to the very bottom rack. While clearing the product out EE was using their left hand to move product and had their right hand on the ground. EE states right hand gave out from under them causing them to fall to the ground. While EE was falling to their ground EE hit their tooth on their rack. EE stated they felt a little pain but thought nothing of it. After finishing their task EE took a few steps and seen the tooth fall out of his mouth. This tooth is a fake tooth that was screwed into EE 's mouth from a previous procedure. Called CVS [REDACTED] Manager [REDACTED] stated the cameras did not show anything.
