What happened
Employee backed the forklift into an upright of the racking system. When the forklift struck the upright the forklift jumped forward and the employee fell from the rear of the lift. The employee 's hand was on the forklift controls and when the employee fell backwards the employee pulled the control causing the forklift to move backwards pinning the employee between the upright and the forklift.
