Before the incident

The CNC Machinist was preparing the [REDACTED] to run a new program. The machine is equipped with an automatic tool changer (ATC). The Machinist executed a command for the machine to change a tool (a face mill).

What happened

The ATC failed to release the tool from the tool holder which caused the machine to fault out and go into Recovery Mode. In Recovery Mode the [REDACTED] must manually release the tool. The [REDACTED] tapped the tool lightly with a rubber mallet to attempt to dislodge it. As the [REDACTED] reached into the machine positioning their left hand beneath the tool for support to prepare for the release they asked a nearby Maintenance Associate to hit the release button. After the associate hit the release button the machine ejected the tool from the holder. According to the [REDACTED] the tool shot out of the tool holder with more force than expected. The force of the tool 's ejection and sharp surfaces lacerated the [REDACTED] 's left thumb and ring finger.
