Before the incident
The employee had walked into work and begun to prepare for the day and then decided to return to their car to retrieve a sweatshirt due to the cold.
What happened
At approximately [REDACTED] on [REDACTED] the EE arrived for their shift and then went to their car to retrieve a sweatshirt. On their way back to the door of the warehouse they slipped on the curb and rocks and fell. Once they got up they noticed that their right ring finger was crooked and swollen. EE went to [REDACTED] where the doctor noted that the finger was dislocated and the doctor put the finger back in its socket and wrapped it in gauze. No medication was administered.
