Before the incident
Operator B (injured employee) was helping fellow operators with Press #5 change over transferring a finished pressed sheet of plastic (80” x 40” x 6.25”) to the material table using a 5-ton gantry crane with a vacuum lift attachment . Once the finished pressed sheet of plastic was transferred and properly placed on the material table Operator A (Lead) and Operator B (injured employee) began to unhook the vacuum lift from the gantry crane hook leaving the unhooked vacuum lift stable on the table on top of the plastic sheet and material table
What happened
Once the finished pressed sheet of plastic was properly set on the material table both operators began to unhook the vacuum lift and left the vacuum lift sitting stable on the finished pressed sheet and material table. Operator B (injured employee) stood out of the line of fire and began to move the hoist block (never raised the hoist block) over to a staged raw material compound bin using a remote pendant. Unbeknown to Operator B the hook was caught on the vacuum lift. Operator B staring at the point of operation (hoist block and hook) moved the trolley about 3-4 feet on the cross girder putting tension on the line. The vacuum lift then began to tip over. Operator B stepped forward into the line of fire trying to stop the vacuum lift (753 lbs.) from falling going in between the material table and staged raw material compound bins. Operator B was pushed backwards by the vacuum lift and into a raw material compound bin and forced to the ground. Operator B was not caught under the vacuum
