Operating a fork truck maneuvering the above said pallet into position.
What happened
Employee was transferring Galvanized steel plates from a wooden pallet supported on the forks of a fork truck to the storage area. When plates were removed more from one side of the pallet than the other; the pallet tilted towards the side in which the employee was standing causing the remaining plates to fall (25 inches) onto the employees left foot. The employee was wearing proper PEE including metarsal shoes safety glasses hearing protection and cut resistant gloves.
