What happened
Employee found the ON OFF adapter that was on the DIL and needed to place it with the original order upon finding the adaptor the was no room on the pallet that the original order was on so the employee went to set it on an open end cone not knowing the tip of the adaptor would go right thru when the employee went to set it down on top of the cone the adaptor went thru and while the employee was still holding onto the parts the weigh of the adaptor carried it down crushing her fingers
