Before the incident
Worker was pouring a concrete placement on level 3 of structure worker had his back against the wire cable perimeter protection.
What happened
Wire rope perimeter protection stanchion was not properly welded to structure with no present 45 degree support on stanchion causing a weak point in perimeter protection. When the worker had his back against the top rail of perimeter protection the improperly welded stanchion broke free causing the wire rope to give way and the worker fell backwards off the building and was caught by another worker who saw the worker falling. Worker was pulled back into the structure by multiple workers.
