Replacing gearbox on DAF tank skimmer

What happened

On [REDACTED] [REDACTED] Employee A was working on installing a new gear box on the DAF tank skimming rake in [REDACTED] with Employee B. Employee A was laying down on the platform on top of foam protectors. As the employees pulled the shaft of the skimming rake up through the bearing the keyway got off center. The employees attempted to turn the gearbox to allow the bearing to slide around the shaft but that caused back pressure to build up. The employees then tried to manually force the skimmer arm to line up the keyway. Employee A reached through an opening in the platform extended his arm and tried pulling on the skimmer arm. As soon as Employee A tried to pull he felt an immediate pop in the lower left abdomen and let out a yell. Employee B then radioed for help. Maintenance co-workers and First Responders arrived on the scene and notified emergency services after the employee reported trouble breathing and difficulty moving. The employee was taken by ambulance to the local hospit
