Before the incident
Employee suffered broken bones in his left hand wrist area when a mold jacket fell on his arm on the weight and jacket transfer on the summit mold line. A couple of maintenance employees were using a come-along to free up a jammed weight and drop it back onto the jacket. The come-along was supported by a pry bar overhead which slipped. The employee had his hand under the weight when it slipped.
