Employee was tightening a Strap on truck (Straight Truck) The chain Anchor portion of the Strap became Lodged Wedged between the Bed and Rub rail causing slack in the chain. The tension of tightening caused the chain to become dislodged suddenly. This caused the load bar to drop and Tomas to twist and fall to the ground.
