What happened
A CSR and a forklift operator were trying to pull two 6 3 4 collars off the second level of a three tier rack. When doing this the forklift operator pulled one too many collars off the rack. As the forklift driver was putting one collar back the CSR was holding the collar they needed to prevent it from rolling off the forks and back onto the rack however the collar being put back didn 't clear the stoppers on the rack causing it to bounce backwards slamming into the fingers of the CSR. At the time of this incident the employee wasn 't wearing impact resistant gloves.
