Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being crushed between the ram and stop baffle wall of the HRB-X baling press due to activation of the baling press while an employee was inside the press cavity clearing excess paper from the forming cavity on 3/2/87. a) Plant, Waste Paper Baling Area: One feasible and acceptable abatement method, among others, to correct this hazard is to establish and enforce an adequate plant lockout procedure which would include the following: 1) Interlock the hopper door with the main power so that the baling press will not operate when the hopper door is open. Arrange the hopper so that when someone is inside the press cavity, the hopper door cannot be closed either from the inside or outside of the cavity. Install a warning sign on the hopper door reading, "Danger- Disconnect and Lockout Power Before Opening This Panel". 2) Open and lockout the main power disconnect for the baling press. 3) Establish a written lockout policy, including procedures to be followed and training for both maintenance and operational employees. 4) Periodically evaluate the program and enforce all its provisions.
