Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
death and/or crushing hazard created by placing a side load on shackle
pins, which could lead to legs opening up:
a) Grandview Surgical Building - A side load was placed on four (4)
shackles used to hoist a 2,000-pound roof top unit, on or about
March 24, 1987.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is the use of the lifting frame method as
recommended by the manufacturers.
