death or serious physical harm to employees in that employees were
exposed to:
a)  Asphalt Plant - Employees were exposed to the hazard of
being run over by Caterpillar Front Loader 980 due to
vehicle having defective brakes.
Among other methods one feasible and acceptable method to
correct this hazard is to repair the brakes on the vehicle
and initiate a preventive maintenance program for all
vehicles to prevent the problem of re-occurrence.
