Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of fractures, crushing and amputation injuries resulting from the operation of the in-house, constructed material elevator in the Cutting Department that had exposed steel beams that when in operation exposed employees to shear points and pinch points, on or about 5/9/88. Among other methods, one feasible and acceptable abatement method to correct this hazard is to enclose the hoistway of the elevator through- out the height of the enclosure. Reference ANSI A17.1-1971. Section 100.1a and ANSI A17.2-1973 Section 101.5.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of fractures, crushing and amputation injuries resulting from the operation of material elevators not equipped with safety devices; including the "in-house constructed" material elevator in the Cutting Department that dropped suddenly after slack developed in the hoisting chain, on or about 5/9/88. Among other methods, one feasible and acceptable abatement method to correct this hazard is to Provide a slack-rope device equipped with a slack rope switch of the enclosed manually reset type which shall cause the electric power to be removed from the elevator driging machine motor and brake if the hoisting ropes become slack, in accordance with ANSI A17.1-1971, Section 210.2.
