Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) S.E. 200 ton Clyde Tower Crane located at the graving dock - Two
major mechanical deficiencies combined with the failure to conduct a
1 to 12 month annual structural inspection of this crane could result
in the collapse of the crane.
One feasible and acceptable abatement method to correct this hazard is
to abate by installing a new drum brake and ratchet and pawl on the
auxillary hoist and conduct a complete structural inspection of this
crane.
Reference ANSI B30.4-1973-4.2.1.3. A complete inspection of this crane
was not performed since November 5, 1988.
b) S.E. 200 to Clyde Tower Crane located at the graving dock -
Reference ANSI B30.4-1973-4.2.3.3(a), A defect disclosed by the
inspection requirements was not corrected before operation of the
crane was resumed.  Cracked hoist drum brake.
c) S.E. 200 ton Clyde Tower Crane located at the graving dock -
Reference ANSI B30.4-1973-4.3.24(c), When the load was suspended for
any considerable length of time a dog or pawl and ratchet or other
equivalent means rather than the brake alone was used to hold the
load.
