Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the drilling cable:
(a) On or about 02/08/04 an employee, while standing on a catwalk directly
under the
drilling cable, was exposed to the hazard of being struck by the drilling
cable when the
"snake grip" attached to the catline and drilling cable separated due to
improper loading
of the drilling cable into the "snake grip".
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to ensure all employees are aware of, understand, and adhere to
the
manufacturer's caution statement and guidelines for the proper
installation and use of a
rope and cable connector grip (snake grip).  Secondly, implement Wolverine
Drilling's
written safety program to ensure employee training in hazard recognition
and avoidance
is accomplished.
