General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely to
cause death or serious physical harm to employees in that employees
were exposed to the hazard of explosion while carrying out autoclave
operations in that:
a- Rubber Department - On or about 2/11/92, Butane Tank Company
Autoclave, where vulcanization operations were being carried
out, the employer had not provided the autoclave with an
appropriate interlocking device on the door in accordance with
ASME Boiler and Pressure Vessel Code, Section VIII, 1968.
Among other feasible and acceptable methods of abatement methods
to correct this hazard are as follows:
1- An adeeuate written Standard Operating Procedure shall
be provided which will instruct all operating personnel in
the proper operation/maintenance procedures of the autoclave.
2- Provide an interlocking device on the door which would
make it impossible to heat or pressurize the autoclave unless
the door is properly closed or to open the door unless the
autoclave is fully pressurized.
