Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. The mud pump was bull plugged preventing release of excessive pressure exposing employees to a struck-by hazard on September 8, 1991 when the four inch hose became disconnected and whipped about striking an employee. One feasible and acceptable method of abatement to correct this hazard is to install a pop off valve on the mud pump to release excessive pressure.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. The four inch hose on the mud pump was not secured to prevent it from whipping about when it became disconnected on September 8, 1991, striking an employee. One feasible and acceptable method of abatement to correct this hazard is to install safety chains on the hose and secure to a solid structure that would hold the hose from whipping.
