Section 5(a)(1) of the Occupational Safety and Health Act of 1970:

The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
On and prior to 11/7/95, employees conducting web thread-up
operations on the Windmoller & Holscher (Olympia Stellaflex)
Printing Press were exposed to pinch points between idler and press
rollers.
Among other methods, the following are feasible and acceptable
methods of minimizing the probability of employee injury from nip
and pinch points.
(1) Develop written thread-up procedures which will eliminate
operator exposure to moving press mechanism in the press roll area.
(2) Train all affected employees on the requirements set forth in
the procedures.
(3) Enforce employee use of the procedures.
(4) Provide interlocks to prevent the activation and movement of
press mechanism while employees conduct web thread-up operations in
the press roll area.
