recognized hazards that were causing or likely to cause death or serious

employees in that employees were exposed to the hazard(s) of falling from
an elevated
platform:
a)  At the construction site - Employees were exposed to fall hazards
while using a forklift
tractor with an unapproved platform to perform work, access higher
elevations, on or about
08/10/99.
b)  At the construction site - Employees were allowed to ride on the forks
of a forklift
tractor, while this equipment was traveling, on or about 08/10/99.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to provide an approved platform designed for this purpose and prohibit
the
practice of
riding on this platform while the forklift tractor is traveling.
