P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of struck by collapse of steel structure: a) At the worksite, manufacturing area, cooker area - Employees were exposed to struck by hazards. Employees were operating a monorail/hoist system that did not have its maximum rated load capacity conspicuously indicated, was not free from detrimental vibrations during normal operation, has not been subject to frequent and periodic inspections nor a preventive maintenance program, on or about 01/08/01. Among other methods, one feasible and acceptable abatement to correct these hazards is: 1. Test/evaluate the monorail system to determine its maximum rated capacity. 2. Perform frequent and regular inspections on the system. 3. Develop and implement a preventive maintenance program. For additional assistance please refer to ASME/ANSI B30.11-1993 "Monorails and Underhung Cranes", Sections 11-1, 11-2 and 11-3.
