Section 5(a)(1) of the Occupational Safety and Health Act of
1970: The employer did not furnish employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees in that employees were exposed to the hazards of being
struck by a collapsing radio tower. On or about 06/06/94,
employees were completing the hoisting of a coaxial cable when
the tower collapsed. After the tower had been constructed, the
lower snatch block had been repositioned due to the construction
of a building next to the tower. Repositioning the snatch block
away from the base of the tower, and not at a guyed section of
the tower, resulted in a concentrated lateral load for which the
tower was not designed.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to follow the manufactures
recommendations for the hoisting of coaxial cable, which is to
either pull at the base of the tower, or at the guyed section of
the tower.

Section 5(a)(1) of the Occupational Safety and Health Act of
1970: The employer did not furnish employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees in that employees were exposed to the hazard of being
struck by a falling gin pole. On or about 06/06/94, employees
were attempting to lower a gin pool from the top of a radio tower
when a cable securing the pole came unhooked, allowing it to fall
approximately three feet. The upper hook had a safety latch that
was defective, and the lower snatch block had the safety latch
removed:
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to follow the inspection procedures
outlined in ANSI, B 30.10-1987. Section 10-1.2.1.2(c)(4) which
requires that hooks shall be inspected for the following items,
latch engagement, damaged or malfunctioning latches. Section
101.2.3(a)
states any conditions disclosed by the inspections
performed in accordance with the paragraphs 10-1.2.2 or 10-1.2.3
shall be corrected by repair or replacement before continuing to
use the hook.
