Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of crushed or broken bones from the unsafe operation of a pipe threader: a)Building #8, boiler house Employees exposed to the hazard of being caught in a pipe threader, were operating a Rigid 300 electric pipe threader, S/N 7592826, which was not provided with an operable foot switch control as required by the American National Standards Institute B208.1-1982 Section 4.4. One feasible method of abatement would be to provide a momentary contact device to control power input to the pipe threading machine. A guarded foot switch is an example of such a momentary contact device. Violation observed on or about 6/8/95.
