Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees:
In that on 1/6/92, an employee conducting sandblasting operations inside
an empty 10,000 gallon fuel tank did not have adequate communication with
employee(s) working outside the tank.
Among other methods, one feasible and acceptable method to correct this
hazard is to assign a trained employee to stand outside the tank and
maintain communication with those working inside and to aid them in an
emergency.
