Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to serious injury because of; trolley step(s) not capable of withstanding the impact o f fully loaded portable blenders; mechanical problems with switches and trolley stops with only quick fixes implemented and standard operating procedures in use at the time of the accident had no documented changes and did not effectively establish "Safe Procedures" for "Unusual Job"/Manual Movement of portable blenders: a) Batch Polymer Area, between stationary blenders Nos. 8 and 9; 3/7/91 AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: a) Add larger surface stops. b) Maintain adequate air compression so as to eliminate manually moving portable blenders. c) Paint the rail/switches so operator(s) can observe if the track is continuous. d) Increase employee training on effective S.O.P. (Standard Operating Procedure) and on any procedural changes as required.
