29 CFR 1926.451(a)(12):  All planking of platforms were not overlapped

a minimum of 12 inches, or secured from movement:
a.   Outside east wall, employees were riding a makeshift
platform on a Lull Highlander rough terrain fork truck, s/n DO1290LN,
and lifted to a height of approximately 16-feet.  Platform was not
physically attached to fork tines and not secured from movement.
Platform was used in lieu of scaffolding that had just been
dismantled.
The employer is not limited to abatement methods suggested by OSHA,
i.e., methods explained are general and may not be effective in all
cases.  Other methods of abatement may be equally or more appropriate.
Ultimate responsibility for determining the most appropriate
abatement
method rests with  the employer, given its superior knowledge of
the specific conditions on its worksite.
