Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were exposed to a potential fall hazard while riding on the manually operated manlifts in the east elevator and the west elevator due to the lack of a car enclosure. Among other methods, one feasible and acceptable method to correct this hazard would be to install a car enclosure on each manlift. (b) Employees were exposed to a potential crushing hazard while working near the manually operated manlifts in the east elevator and the west elevator due to the lack of guardrails around the base. Among other methods, one feasible and acceptable method to correct this hazard would be to install guardrails around the base of each manlift. (c) Employees were exposed to a potential fall hazard in the west elevator while operating the manually operated manlift due to the damaged and frayed rope. Among other methods, one feasible and acceptable method to correct this hazard would be to install a new rope.
