Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of amputation:
(a)  On or before February 28, 1998, in the pill press area, the selector
controls and the
physical barrier guards for the #4 hydraulic pill press were not
controlled or supervised
by the employer.  The press operators had control of the "key" which
allowed them to
open the plastic guard and enter the point-of-operation area.  Also, the
selector modes
of operation were not controlled by management.  American National
Institute Standards
(ANSI) B11.2-1982, paragraph 5.3.4.1 (3) states:  "Barrier guards shall be
fastened so
that they are not readily removable by the operator and/or interlocked
with the press
control to prevent slide(s) motion if the guard is removed or opened."
Abatement Note:  Among other methods, one feasible and acceptable
abatement method
to correct the hazard is to ensure that the barrier guards are not readily
removable by the
operator and the interlocks are all in proper working order and checked
for proper
operation at least at the start of every shift by management.

on hydraulic pill press #4 descended with a pressure of approximately 82
pounds.
American National Standards Institute (ANSI) B11.2-1982, paragraph 5.2.1
(4) states:
"The following are considered to be similar to point-of-operation hazards:
any other
pinch point in or near the point-of-operation."
Abatement Note:  Among other methods, one feasible and acceptable
abatement method
to correct the hazard is to ensure that the descending pressure on the
safety gate does not
exceed 5 pounds.

hydraulic pill presses establishing guidelines for operation and care of
the presses.
American National Standards Institute (ANSI) B11.2-1982, paragraph 4.1.1
states:  "It
shall be the responsibility of the manufacturer to furnish written
instructions with the
hydraulic power press to establish guidelines for the operation and care
of the press."
Abatement Note:  Among other methods, one feasible and acceptable
abatement method
to correct the hazard is to ensure that up-to-date guidelines and
operating instructions are
written, published and accompany each hydraulic pill press used in the
United States.

inspections of
the hydraulic pill presses to ensure that all its parts, auxiliary
equipment, and safeguards
were in a safe operating condition and adjustment.  There was no program
of periodic
inspections on #1, #4, and #5 hydraulic pill presses to ensure that the
safety interlock
mechanism was working and functioning properly.  American National
Standards
Institute (ANSI) B11.2-1982, paragraph 4.3 states:  "It shall be the
responsibility of the
employer to establish and follow a program of periodic inspections of his
hydraulic
power press to ensure that all its parts, auxiliary equipment, and
safeguards
are in a safe
operating condition and adjustment."
Abatement Note:  Among other methods, one feasible and acceptable
abatement method
to correct the hazard is to ensure that a "hands-on" inspection by company
management
be made of the operation and safeguards of each hydraulic pill press be
made at least
once per shift.

the operators in the safe method of work before allowing the operation of
the hydraulic
pill presses in the facility.  Press operators were using improper
cleaning practices when
reaching into the die area and cleaning out the dies with compressed air
and operating
the hydraulic pill presses without the use of all guards in place.
American National
Standards Institute (ANSI) B11.2-1982, paragraph 7.1 states:  "The
employer shall
instruct and adequately train the operators in the safe method of work
before allowing
operation of any hydraulic power press covered by this standard.  The
employer shall
ensure by adequate supervision that correct operating procedures are being
followed."
Abatement Note:  Among other methods, one feasible and acceptable
abatement method
to correct the hazard is to ensure that each and every press operator is
adequately trained
over every aspect of the operation of these hydraulic presses including
the review of all
manuals and written materials.

the press next to the start button had been disconnected and the stop
button on the control
panel could not be activated in an emergency in that the plastic door
covered access to
the button when the side guard of the press was open.  American National
Standards
Institute (ANSI) B11.2-1982, paragraph 3.4.1.1 states:  "A means of
stopping
the motion
of all press slide(s) shall be provided.  Actuation of the stop control
shall deactivate
immediately all press controls.  The stop control shall override any other
control."

to correct the hazard is to replace the stop button on the front of the
press.
