Section 5(a)(1) of the Occupational Safety and Health Act of 1970. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards associated with working in confined spaces: (a) Building 257 - Employees who must enter the boilers on the fire side and clean them out are exposed to the hazards associated with working with caustic soda and respirable dust; on or about 9/25/92. (b) Building 257 - Employees who monthly enter the incincerators to clean them out are exposed to the hazards associated with respirable dust; on or about 9/25/92. (c) Building 102 - Employees who monthly enter water treatment tanks are exposed to hazards associated with working with hydrochlorine acid; on or about 9/25/92. ABATEMENT NOTES: Among other methods, one feasible method to reduce this hazard is to establish and enforce adequate Confined Space Entry Procedures as required by 29 CFR 1910.146 "Permit Required Confined Spaces for General Industry", printed in the Federal Register on January 14, 1993, and becomes effective April 15, 1993. Guidance on Confined Space Entry may also be obtained from the American National Standards Institute (ANSI), Safety Requirements for Confined Spaces, ANSI Z117.1-1989.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being pulled into and crushed by the rotating parts of a pipe threading machine: (a) Building 101, Basement, Machine Area - Employees operating a Ridge 400 pipe threading machine which was activated by a toggle switch that would not automatically shut the machine off if its operator was inadvertently caught up in its rotating parts; on or about 9/23/92. (b) Building 101, North Corridor Area 2-S - Employees operating a Ridge 400 pipe threading machine which was activated by a toggle switch that would not automatically shut the machine off if its operator was inadvertently caught up in its roating parts; on or about 9/24/92. ABATEMENT NOTES: Among other methods, one feasible method to reduce this hazard is to establish and enforce adequate safe operating procedures. Procedures such as those stipulated by American National Standard for Portable Pipe Threading Machines (ANSI B208.1-1982). Elements essential to an acceptable safe operating procedure include at a minimum: 1. Installation of a momentary contact device (deadman foot switch) to control power input to the machine. This device would automatically stop the machine and the rotating pipe if the operator's foot was removed from the foot switch for any reason.
