Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the end cap on a pressurized
12 inch steel gas
line.
(a)  Location:  Jobsite, New Ocean and Eastern Ave., Lynn, MA;
On or about 12/14/95, an employee was seriously injured when an improperly
attached
end cap on a steel gas line under 90 psi of compressed air blew off
hitting the employee
in the head.
Among other methods of abating this condition would be to follow the
manufacturer's
recommendations for installation procedures of the end cap and/or to bleed
the pressure
from the line prior to removing back-fill.
