Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
Mill employees are required to clean the roofs of the Mill House on a
monthly basis without fall
protection. This task, on occasion, included the truck drivers. The
employer has one safety
harness on the premises which was used by one of the many employees that
were assigned to
clean the roof leaving the other employees without fall protection. The
roof at the mixer one (1)
area had a standard railing along the open sides to protect employees on
the roof from falling,
however the roofs of the Burr, Silage, Commodities and Corn areas did not.
This condition
was exacerbated in that some of the roofs are sloped. This condition
exposed employees to the
hazards of falling from a height of up to twenty-two feet, including but
not
limited to:
contusions, concussions, bone fractures and death.
Among other methods, feasible and acceptable abatement methods to correct
this hazard include:
1) The installation of standard railings along all edges of the elevated
surfaces and
platform on which employees work.
2) The use of fall protection harnesses and lifelines for all employees
working on
elevated surfaces and platforms.

The employer did not protect employees from the hazards associated with
unguarded chains and
sprockets.
a) The dual chains and sprockets in the molasses pit near the reduction
gear were not guarded.
The chain and sprocket were found to be less than seven feet from the
floor.
b) The unguarded dual chain and sprocket located near the electric motor
in the molasses pit
were not guarded.  The chains and sprockets were found to be less than
seven feet from the
floor.
The above condition exposed employees to "caught in" hazards from the nip
points of the
unguarded chain and sprocket, including but not limited to bone fractures
and amputations.
Among other methods, feasible and acceptable abatement methods to correct
this hazard include:
1)  Guard the chain and sprocket with a metal covering to prevent
employees from
coming in contact with the "nip point" of the unguarded chain and sprocket.
2) Lockout/tagout the energy source to the molasses pump to prevent
accidental start up.rt

The belt and pulley located in the molasses pit was not guarded to prevent
employees from
coming in contact with it.  The pulley and belt was measured at less than
seven feet from the
floor. This condition exposed employees to the hazards associated with
caught in hazards from
the in-going nip points of the belt and pulley including, but not limited
to, bone fractures and/or
amputations.
Among other methods, feasible and acceptable abatement methods to correct
this hazard include:
1)  Guarding the belt and sprocket with a metal covering to prevent
employees from
coming in contact with the nip points.
