Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a)The hazard of being struck-by and/or crushed by a suspended load due to
obstructed operator vision caused by damaged to the forward glass of the
Mantis 3612, #BDF-009, mobile hydraulic crane.  One feasible and
acceptable abatement method, among others, to correct this hazard is to
replace the operator's forward cab glass as specified by paragraph 1.8.1.b
of the American National Standard, standard B30.15-1973, Mobile
Hydraulic Cranes.
