ORS 654.010:  The employer did not furnish employment which was
safe and healthful for employees, and did not adopt and use such
practices, means, methods and processes as were reasonably
necessary to protect the life, safety, and health of the employees
as evidenced by:
(a) Training in the recognition of basic safety and health hazards
inherent in the assigned job was not provided.
(b) The employer did not inspect, cause inspection of or require
documentation of inspection of all worksites where workers were
assigned to determine potential safety and health hazards of those
assignments.
(c) A job safety analysis was not performed for all job assignments
to identify hazards and the means to be taken to control or
eliminate worker exposure.
