Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:
Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico:
El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
At the establishment: It were observed five (5) instances in which
fissured sections on
the ceiling structure in the second level of the building where Fajardo's
Regional
Ambiental Health Office is located, which posed the hazard of traumas and
fractures,
from slippings and falls on rainy days:
a. At the office of the regional director secretary;
b. At the collector's office;
c. At the inspector's offices (men/women);
d. At the secretaries office and,
e. At the walkway.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to restore anew the ceiling with concrete and sealing compound.
En el establecimiento: Se observaron cinco (5) casos en los cuales
secciones resquebradas
en la estructura del techo en el segundo nivel del edificio donde se ubica
la Oficina
Regional de Salud Ambiental de Fajardo los cuales constituyeron un riesgo
de traumas
y fracturas, por resbalones y caidas en dias lluviosos:
a. En la oficina de la secretaria del director regional;
b. En la oficina de la recaudadora;
c. En las oficinas de los inspectores (hombres/mujeres);
d. En la oficina de las secretarias y,
e. En el pasillo.
Entre otros metodos, un metodo accesible y aceptable para corregir este
riesgo es reparar
de nuevo el techo cemento y compuesto sellador.

At the establishment scretaries office entrance: An on/off light control
110/120
volts
interrupter at 56 inches from the floor level was observed in which
percolated pluvial
entered through the wireway, accumulated on its base with runoff to the
floor, exposing
the employees to the hazard electric shock and or electrocution.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to seal the wireway and renew the damaged interrupter.
A la entrada de la oficina de las secretarias del establecimiento: Un
interruptor de control
de luz encendido/apagado, de 110/120 voltios, a 56 pulgadas del nivel del
piso, fue
observado en el cual pluvial percolado entro atraves del conducto de los
cables, se
acumulo en la base con escurrimiento hacia el piso, exponiendo a los
empleados al riesgo
de sacudida electrica y/o electrocucion.
Entre otros metodos, un metedo accesible y aceptable para corregir este
riesgo
es sellar
el conducto de los cbles y reemplazar el interruptor danado.ptor

estaban expuestos
a:
At the establishment: It were observed three (3) instances in which
concrete sections of
the ceiling of a square feet size menace to collapse and fall from ten
(10) feet height;
exposing the employees to the hazard of head and body traumas and or
serious injuries:
a. At the office of the regional director secretary sanitary facility;
b. At the collector's office sanitary facility and,
c. At the secretaries office sanitary facility.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to restore anew the ceiling with concrete.
En el establecimiento: Se observaron tres (3) casos en los cuales
secciones de
empenetado de cemento del techo interior de un pie cuadrado en tamano
amenazaban con
colapsarse y caer desde diez (10) pies de altura; exponiendo a los
empleados al riesgo
de traumas y lesiones serias de la cabeza y el cuerpo:
a. En la facilidad sanitaria de la oficina de la secretaria del director
regional;
b. En la facilidad sanitaria de la oficina de la recaudadora y,
c. En la facilidad sanitaria de la oficina de las secretarias.
Entre otros metodos, un metodo accesible y aceptable para corregir este
riesgo es reparar
de nuevo el techo interior con hormigon.

Seccion 7.3.1. El patrono no proveyo empleo y un sitio de empleo libre de
riesgos reconocidos
que esten causando o que puedan causar muerte o dano fisico a los
empleados, por cuanto
empleados estaban expuestos a:
At the establishment: The luminaires on the second floor walkway were not
functioning,
creating a dark space where the employees were exposed to the hazard of
slippings and
or strippings with the resultant of body traumas and or injuries.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to replace the luminaires or to provide a separate perimeter
lighting system.
En el establecimiento: Las luminarias en el pasillo del segundo piso no
estaban
funcionando, creandose un espacio obscuro donde los empleados estaban
expuestos al
riesgo de resbalones y/o tropezones con la resultante de traumas y/o
lesiones del cuerpo.
Entre otros metodos, un metodo accesible y aceptable para corregir este
riesgo es
reemplezar de nuevo las luminarias o proveer un sistema separado de
alumbrado del
perimetro.
