Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of falls within the personnel platform:
a) The personnel platform was not equipped with a grab rail installed
inside the entire perimeter of the personnel platform.
Among other methods, one feasible and acceptable method to correct the
hazard is to install a grab rail inside the entire perimeter of the
personnel platform, such is required by 1926.550(g)(4)(ii)(B).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of being struck by falling object, such as but not limited to
the bridles, master link and headache ball, when the personnel platform
is landed:
a) The employees riding in the personnel platform were not
provided overhead protection other than the use of hard hats
when exposed to falling objects.
Among other methods, one feasible and acceptable method to correct this
hazard is to provide the personnel platform with overhead protection
such as required by 1926.550(g)(40)(ii)(F).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of collapse of the personnel platform due to possible over
loading:
a) The personnel platform was not conspicuously posted with
a plate or other permanent marking which indicates the weight
of the platform and its rated load capacity or maximum intended
load.
Among other methods, one feasible and acceptable method to correct the
hazard is to conspicuously post a plate or other permanent marking with
the rated load capacity or maximum intended load as required by
1926.550(g)(4)(ii)(I).
