Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Microwave tower, west side parking lot of West Penn Power Company,
Charleroi, PA; employees constructing a microwave tower were
permitted to ride the headache ball, hook and/or the load, exposing
themselves to a fall hazard of 80 feet.
A feasible and useful method to correct this hazard, among other methods
is to prohibit the employees from riding the load, headache ball or
hook.

a) Truck mounted winches, west side parking lot, a complete inspection
of the hoist was not performed at intervals as generally defined in
ANSI B30.7 1971-72.1.1.b.2 depending upon its activity, severity of
service and environment or as specifically indicated below:
1) The notches on the winch clutch locking system was worn
2) The clutch pull rod was bent.
3) The dogs in the clutch engaging system was worn.
4) A wad of grease was on a section of the dogs when engaged.
A feasible and useful method to correct this hazard, among other methods
is to replace the dogs, notch section and install newer rods to prevent
their bending.

a) A truck mounted power winch west parking lot, a thorough inspection
of all ropes were not made at least once a month and a full written,
dated and signed report of rope condition kept on file and a
determination made as to whether future use of the rope would
constitute a safety hazard. The 3/8 inch wire rope 1500 ft. long
used to lift material was not installed in the wedge socket connected
to the hook so that the pulling part was directly in line with the
clevis pin. This will produce a sharp bend in the rope as it enters
the socket.
A feasible and useful method to correct this hazard, among others, is
to replace the dogs, notch section and install heavier rods to prevent
their bending.
