Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about June 6 and 7 employees were exposed to a high level of radio frequencies (RF) in excess of that allowed by American National Standards Institute Standard (ANSI C95.1-1982) Radio frequency Guide. One possible emthod of abatement would include establishment of a radiological protection program to identify areas of high RF and "turn-off" the RF source when employees must be in these areas.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Hazard of fall on or about June 6-7, 1991 - Employee(s) were climbing the outside frame of the 1560 ft. high Kline Tower without fall protection. Employee(s) were not using the fixed ladder of the tower. Employees were in the process of erecting a hoist on the tower. Among other methods, one feasible and acceptable abatement method to correct this hazard is employees shall use the ladder safety system provided on the Kline Tower.

Employees were riding the 5/16 inch cable of the job made hoist without fall protection. Employees were repairing Strobe lights on the tower. Tower site. Among other methods, one feasible and acceptable abatement method to correct this hazard is employees shall use a hoist machine approved by the manufacturer. Employee's shall use a life line and safety clamp or grip to prevent falls. Employees shall use a tower elevator to access the tower.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazard of fall: (a) Employee(s) were riding the 5/16 inch wire rope of the job made hoise which did nto comply with the following: 1). The hoise machine was not provided with a deadman control switch, that when released would promptly deactivate the engine and stop the hoise drum rotation thereby stopping the travel of the hoist cable. 2). The hoise machine was not provided with a line speed indicator located within clear view of the operator. 3). The hoisting system was not equipped with limit switches to automatically prevent overtravel at the top supporting structure and at the base. 4). The hoist machine was not designed, installed, and located to provide proper winding or spooling of the rope on the drum. 5). All electrical circuitry was not weatherproof. 6). The hoist machine wa snot provided with a audible and/or visual overspeed indicating alarm that would activate before the line speed exceeds 375 ft. per minute (10% averaged) when transporting personnel. 7). The hoist was used to hoist materials and personnel simultaneously. 8). Where the hoist and/or hoist operator was subject to falling objects, much as tools, materials, or debris, they were not adequately protected from such hazards. 9). The hoisting rope was not attached to the personnel cage by a positive conneciton such as a screw pin shackle. The pin was ot mored or drilled to accept a cotter key or bolt to prevent accidental disengagement. 10). The hoisting rope was ot brricaded between the hoisting machine and the footblock and protected where the hoisting rope was subject to falling material or debris, it was not protected from such hazards. 11). An independent lifeline and a belt (body belt) or harness, conforming to applicable regulation, was not provided and used by employees bieng transported by the nonguided hoist. 12). Following the assembly and erection of the hoist system and before it is put into service to transport personnel, and inspeciton and test of all function and safety devices wre ot made. 13). Records of the initial, weekly, and quarterly inspections were not maintained. Inspections records were not available for review. Among other methods, one feasible and acceptable abatement methods to correct this hazard is: 1). Comply with above listed requirements. 2). Use the ladder safety system currently on the Kline Tower.
