Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential fall hazard: (a) Employees were exposed to a potential fall hazard while operating the electric manlift in the main elevator due to the door not latching properly. The door had an interlocking device but was inoperable. Among other methods, one feasible and acceptable method to correct this hazard would be to install an interlocking device that would prevent the door from opening when away from any of the landings. (b) Employees were exposed to a fall hazard while operating the manually operated manlift in the concrete facility due to the lack of a car enclosure on the manlift car. Among other methods, one feasible and acceptable method to correct this hazard would be to install a car enclosure on the manlift. Among other methods, one feasible and acceptable method to correct this hazard would be to install some type of fall protection at the load- out area.
