Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard(s) of being struck-by collapsing
structural steel
members due to the inadequate construction of the concrete columns
supporting them.
At the job site, on or about 02-23-98 through 02-25-98, three employees
were exposed to the
hazard(s) of being struck-by structural steel members due to the
inadequate construction of
ten concrete columns.  These 14-inch in diameter by 45-inch long columns
were constructed
without four sections of vertical #4 REBAR connected by #3 ties at
eight-inch intervals.
These columns were not centered or connected to ten three-foot square by
24-inch thick
concrete footings below through the use of two #4 dowels with an equal
number of six-inch
hooks.  Additionally, an unauthorized steel, nine-inch by six-inch anchor
or base plate was
positioned within all ten concrete columns.  Twenty 12-inch long
all-thread rods were also
used instead of standard anchor bolts.  Four steel cross-sections
consisting of two tapered
beams and four interconnected rafters with associated girders and purlins
were placed on
eight of the non-reinforced and improperly supported concrete columns.
The structural steel
collapsed on or about 02-25-98 resulting in the serious injury of one
employee.
One method of abatement to correct this apparent hazard is to follow the
registered structural
engineer's certified drawings depicting the proper method of constructing
the concrete
columns as required by sound construction principals.
