Citation: 1   Item: 1   Type of Violation: SERIOUS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which was free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
(a) CONOCO refinery, Billings, Montana:  On or about 14 December 1998, and
at times
prior thereto, employees were exposed to traumatic amputation while
pigging the Coker
Furnace lines using high pressure water lines with a valve that was not
rated for the
pressures and conditions applied.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to ensure that all parts including, but not limited to valves,
are rated for the
pressures and service to which they will be exposed, and that they are
protected from
overpresurization by a safety relief device.
