Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the potential hazard of severe electrical shock and or
electrocution: The
following conditions contributed to the existence of these hazards:
a) CP/V Pro Surveyor: 110-120 volt electrical outlets located in
potentially wet/damp
locations near the Galley sinks were not of the Ground Fault Circuit
Interrupter (GFCI) type and
the controlling branch circuit was not equipped with a GFCI type circuit
breaker at the panel.
One feasible means of abatement, among others, is to install a GFCI type
circuit breaker in the
control panel.
b) CP/V Pro Surveyor: A 110-120 volt electrical outlet located at the wash
basin in the
Galley head was not of the GFCI type and the controlling branch circuit
was not equipped with
a GFCI type circuit breaker.
One feasible means of abatement, among others, is to replace the outlet
with a GFCI type
receptacle.

c) CP/V Pro Surveyor: In the Main Deck Power Panel; circuit breaker #21
was missing
and energized electrical wiring, a terminal screw, and busbar were visible
and accessible through
the opening.
One feasible means of abatement, among others, is to insert a blank
coverplate into the opening.
d) CP/V Pro Surveyor: In the Engine Room Lighting Panel; circuit breakers
#23 and 24
were missing and energized wiring, terminal screws and the busbar were
visible and accessible
through the openings.
One feasible means of abatement, among others, is to insert blank
coverplates into the openings.
e) CP/V Pro Surveyor: In the Engine Room; a 110-120 duplex outlet was
missing its
coverplate and energized wiring and terminal screws were visible and
accessible.
One feasible means of abatement, among others, is to install the
coverplate.
